| BOOKING ALLEGIANT: |
|
There is a $15.00 booking fee for all AIR-ONLY and PACKAGE tickets purchased through an Allegiant call center. Tickets booked online will not incur this fee. This fee is per person, per segment, regardless of the number of segments or passengers booked in an itinerary. The $15.00 booking fee will be waived for: Unaccompanied minors, customers with special needs (SSR’s), non-revenue, vouchers, and group bookings. In addition, there is an $14.99 convenience fee per traveling customer when booked via www.allegiant.com or an Allegiant call center. These charges are non-refundable.

| UNACCOMPANIED MINORS: |
|
To ensure the well-being of the child while meeting the service and expectations of our customers, effective 11/1/2009, Allegiant will no longer accept reservations for fare-paying and/or any non-Allegiant employee dependant for transport as Unaccompanied Minors (UM) or unescorted children under the age of fourteen (14) years.
Children thirteen (13) and younger must be accompanied by an adult fourteen (14) years or older. Children between the ages of fourteen (14) years and seventeen (17) years can be booked online with no extra fees. Special supervision for ANY fare-paying travelers will no longer be offered as of 11/1/2009.
When Allegiant issues a Weather Advisory for either the departure or arrival city, and/or the potential for a flight to be diverted enroute, we will recommend minors do not travel and will rebook at no fee. However, if a minor decides to travel, they will continue to assume all responsibility.

| GROUP BOOKINGS: |

Please email groups@allegiantair.com with group booking requests. Include your name, contact information as well as the number in the party (A group consists of 10 or more passengers), requested dates, itinerary and hotel request.

| COMMISSIONS: |
|
Allegiant pays 10% commission on air/hotel packages for:
- Las Vegas (LAS)
- Orlando/Sanford (SFB)
- Tampa Bay/St. Pete (PIE)
- Phoenix-Mesa (IWA)
- Fort Lauderdale (FLL)
Allegiant does not pay commission on AIR ONLY bookings, attractions; NASCAR packages; transfer tickets; seats; checked bags or travel protection.
Commission checks are issued once a month on TRAVEL COMPLETED the previous month. Said checks are generally mailed out of Las Vegas around the 9th-10th and agencies receive between the 12-15th of each month.

| ALLEGIANT INDUSTRY TRAVEL PROGRAM: |

Valid IATAN and/or CLIA members plus 1 companion may travel on Allegiant's Industry Rate of $35.00 + Tax, per person, each way. Travel may be confirmed 5 days prior to the date of departure. At this time, the return trip will not be confirmed, unless travel is within 5 days of booking.
Allegiant offers an industry travel rate of $35.00 per person plus taxes each way, good for one IATAN and/or CLIA cardholder and companion traveling on the same itinerary. To take advantage of this rate, contact our Reservation Department no earlier than 5 days prior to the date of departure. At this time, the return trip will not be confirmed unless travel is within 5 days of booking. Ask the agent if the travel industry rate is available on your requested flight dates. If available, the originating flight will be confirmed at this time. Please note the industry rate is inventory controlled and does not have last seat availability. It is not always possible to accommodate travel agents on our most popular travel dates.
